We do not disclose data about identifiable individuals to our advertisers, but we may provide them with aggregate information about our users (for example, we may inform them that 400 women between 30 and 45 have clicked on their advertisement on a specific day). We may also use such aggregate information to help advertisers target a specific audience (for example, men in a specific location). We may make use of the personal information we have collected from you to enable us to display our advertisers’ advertisements to that target audience.

CONSENT TO THE PROCESSING OF INFORMATION IN CANADA

We store your personal information on servers located in Canada. By using and accessing our Site, users agree that their personal information can be stored and processed in Canada. 

 DATA PROTECTION

Sensitive data is protected at rest via the AES 256 family of algorithms. Physical locations are protected via GCP Canada and encrypted and managed by google. Data in transit is protected via the latest version of SSL for servers and clients’ connections. TLS 1.2 and 1.3 are leveraged as well as signed SHA256 certificates. Additionally, a custom encryption layer has also been deployed as a defence-in-depth approach.
WholisticDr uses SSL for HTTPS (green and no warnings/error) and all service and network traffic between users and servers use these protocols.
WholisticDr uses ISRG PKI services provided under the brand/trademark "Let's Encrypt". ISRG uses HSMs conforming to FIPS 186-4, capable of providing random number generation and onboard creation of at least 2048-bit RSA keys and at least 384-bit ECDSA keys.

CHILDREN UNDER THE AGE OF 13

Our Website is not intended for children under 13 years of age. No one under age 13 may provide any personal information to the Website. We do not knowingly collect personal information from children under 13. If you are under 13, do not register on the Website, use any of the interactive or public comment features of this Website, or provide any information about yourself to us, including your name, address, telephone number, email address. If we learn we have collected or received personal information from a child under 13 without verification of parental consent, we will delete that information. Appointment Cancellation and No-Show

Appointments may be cancelled by you, with a refund for any payments collected online through our Platforms up to 24 hours in advance of the date of the appointment. Beyond that point and out of consideration and respect of our practitioners’ time, if a patient does not show up for a scheduled appointment, the patient will be charged the full price of the scheduled visit.



Privacy of Personal Health Information - Frequently Asked Questions

Acemanage Smart offers an integrated Virtual Visit Solutions (Video and Message) and Electronic Medical Record (EMR) suite of services using the software platform known as WholisticDr. WholisticDr is used by licensed physicians and health care practitioners within Canada and the United States. The product allows:

  • Patients to register and book appointments with a physician or health care practitioners.

  • Physicians or health care practitioners can manage patient information and appointments as well interact and consult with patients including messages, phone, and video calls.

  • Physician or health care practitioners’ staff can perform patient triage and manage patient information including insurance and pharmacy prescriptions.

Patients are interested in access to their health information, and in digital health opportunities which are often in real-time and enabled by internet connectivity. Engaged patients can realize greater health benefits. Common internet-enabled initiatives include patient online access to portals, email between patients and providers, appointment booking by patients, and appointment reminders sent to patients by text, phone, or email. When custodians collect health information, they have the responsibility to protect the access, privacy, and security of that health information. This responsibility is guided by the professional college standards of practice and provincial privacy legislation, including Ontario’s Personal Health Information Protection Act (PHIPA).

How do I access or request a copy of my health records?

Patients have access to limited amounts of their own information using the WholisticDr Patient Portal. Otherwise, only your healthcare provider has access to your health information. While you are actively receiving care, you should ask your healthcare provider for information that you want to know about your diagnosis and treatment. Contact your healthcare provider to request access to your health information.

What should the request include?

A written request for health records must include your name, date of birth, mailing address, and the type of information you need. The request must be dated, signed with your signature, and witnessed and signed by one other person.

How do I request my records to be released to another individual?

Whether you want your records released to a relative, friend, family doctor or another institution, you must submit a signed consent giving your healthcare provider authorization to release your information. The consent is valid for 90 days and must be dated and witnessed. Contact your healthcare provider to request the release of your health information.

Does a youth under the age of 16 need parental consent?

Children of any age are presumed to have the capacity to consent to the collection, use and disclosure of their personal health information. If there is a conflict between the child and parent, the capable child's decision prevails with respect to the consent. To be capable of consenting, a patient must be able to understand:

  • The information needed to make a decision on whether or not to consent to the collection, use or disclosure of personal health information; and

  • The consequences of giving, withholding or withdrawing consent.

    What happens if I am inquiring about records for a deceased patient?

    To obtain records for individuals who are deceased or incapable of signing a consent, proof of executorship or legal signing authority must be submitted with the request. Does it cost me anything to request my personal health information? There are processing fees associated with accessing (viewing) or requesting a photocopy of your personal health information. A prepayment fee may be required by your healthcare provider before accessing or receiving a copy of your personal health information. Please contact your healthcare provider directly. What information does my healthcare provider collect and maintain on WholisticDr? When you create your personal account on WholisticDr, you provide your name, date of birth, address, Health Card Number. Information relating to previous health problems, the record of your visits to the hospital and what health care we provide to you during those visits are examples of your health information.
